Rocky Mountain Day Service provides residential supports through Host Home, Family Caregiver and PCA settings and community – based services through SCC, Community Connector, and Mentorship services.

Rocky Mountain Day Service will work with each person to design and lead their own person – centered plan to maximize independence and maintain health and safety. Services and supports will vary according to the person’s own plan and support needs, but may include the following:

  • Community Integration

    Refers to assimilating and welcoming people with disabilities into the larger community.  Community integration enables persons with disabilities to fully participate in life at the same level as nondisabled individuals.

  • Skill Development

    Identifies the gaps and develops the skills which enable the person to achieve their goals.  The recognition, practice, and internalization of skills towards improved execution of skills.

  • Socialization

    The activity of mixing socially with others.  The process of learning to behave in a way that is acceptable to society.

  • Healthy Living

    Supporting the individual so both physical and mental health are in balance or functioning well together in the person.

  • Activities of Daily Living

    (ADL’s) focus on the individual’s self-care abilities, such as grooming and hygiene, preparing meals, and using public transport.  We evaluate the extent to which the individual is capable of initiating and participating in activities independent of supervision or direction.

  • Education and Advocacy

    Ensuring that individuals with disabilities receive the services and accommodations they need, and that parents know their rights.

  • Medication Administration

    RMDS provides supports to individuals receiving services: in the use of prescription and non-prescription medications which simultaneously protect the health and safety of individuals receiving services.  Employees and contractors assisting individuals with medications will receive training from an Approved Training Entity to establish them as a qualified medication administration person (QMAP).

  • Assistance with Medical Providers

    RMDS shall provide sufficient oversight and guidance to ensure that the health and medical needs of individuals receiving services are addressed.

  • Personal Finances

    RMDS will support the person to access and manage their own funds and ensure financial accountability for the person’s financial assets.  RMDS will support each person with an individualized fund management plan to include assistance with establishing financial systems (e.g., savings/checking accounts, debit cards, cash, etc.) and tracking tools.

  • Behavioral Support

    This is a plan that assists an individual in building positive behaviors to replace or reduce a challenging/dangerous behavior.  This plan may include teaching, improved communication, increasing relationships, and using clinical interventions.

  • Transportation

    RMDS provides non-medical transportation for clients that are eligible through our day program.
